The Nason Hospital Health Agency is a Medicare and JCAHO certified home care agency administered by the Nason Hospital to provide caring, professional services at home. Services provided are quality skilled nursing, physical therapy, speech pathology, occupational therapy, medical social services, home health aide, Healthy Beginnings program, enterostomal therapy and multi-disciplinary hospice care.

HOME HEALTH AGENCY

Nason Hospital Home Health Agency is Medicare and JCAHO certified to provide quality patient care services (skilled nursing, home health aide, physical therapy, speech pathology, occupational therapy and medical social services) to homebound patients.

Eligibility: Homebound, under the care of a physician and demonstrating a skilled need.

HOSPITAL SOCIAL SERVICES

Services are primarily focused on helping patients and families to cope with the stresses often associated with illness and hospitalization. This includes emotional support/counseling and advocacy as well as assistance with practical discharge planning concerns, i.e., home health care, hospice care, medical equipment, other in-home assistance and transfer to nursing or rehabilitation facilities. Information and referral to community programs.

Eligibility: Must be a patient.

NASON HOSPITAL HOSPICE

Nason Hospital Hospice is Medicare and JCAHO certified to provide multi-disciplinary team services to terminally ill patients and their families. Home care services are provided by Nason Hospital Home Health Agency and acute care and respite services are given by Nason Hospital. The multi-disciplinary team consists of physicians, nurses social workers, a chaplain, home health aides, speech, physical and occupational therapists and volunteers.

Eligibility: Terminally ill, accept Hospice philosophy and an available caregiver.
