Blair County Healthy Community Partnership

MISSION
To enhance the quality of life in Blair County through a long-term strategy so that healthy behaviors are practiced by its citizens.

VISION
For Blair County to be a community in which beliefs, practices and services exist that value, nurture, protect, and empower individuals to achieve their maximum potential through proactive, inclusive, collaborative efforts.

THE PARTNERSHIP
The
Blair County Health and Welfare Council was approved by the Pennsylvania Department of Health as a Community Partnership with the goal of implementing the State Health Improvement Planning (SHIP) process in Blair County. To accomplish that goal the Health and Welfare Council established the Blair County Healthy Community Partnership [BCHCP] as a standing committee of the Council.

The BCHCP works collaboratively with local health, education, human services, criminal justice, and other organizations to improve the health status of residents of Blair County by addressing the root causes of health problems based on an assessment of the county’s health improvement priorities. As a result of that assessment, the Partnership will develop prevention and intervention strategies to address those causes.

The Blair County Healthy Community Partnership adopted the six (6) chapters from the State Health Improvement Plan as functional committees. The committees address different issues in Blair County by assessing the health improvement priorities of each area of concern. Photo

The six committees are:

• Communicable Diseases
• Chronic Diseases
• Family Health
• Violence and Injury
• Environmental Health
• Service Delivery System

COORDINATING COUNCIL
The Coordinating Council guides the work of the Partnership in non-Partnership meeting months.  The Coordinating Council consists of the Chair, Vice-Chair, and Secretary of the Partnership; representatives of all Blair County hospitals; all funders of the Partnership; Chairs of all Sub-Committees; a representative from the State Health Department; President of the Health and Welfare Council; and the Treasurer of the Health and Welfare Council.  The Coordinating Council meets at 8:00 a.m. on the fourth Friday of February, April, August and October.

MEMBERSHIP INFORMATION
The Blair County Healthy Community Partnership is a Standing Committee of the Blair County Health and Welfare Council.The full Partnership membership meets twice annually.  The Annual meeting is held in June of each year to elect a Vice-Chair and Secretary; to hear an annual report; and to conduct any other business pertinent to the Partnership.  The other full Partnership meeting is held immediately following the January Health and Welfare Council meeting.
